Baxter Healthcare provided financial support for the creation and distribution of these resources through support to the healthcare communications agency, The JB Ashtin Group, Inc., including amounts for each member of the Global Forum to attend up to 4 meetings, and for publication costs. Meetings were held on 29 November 2012 in Vancouver, British Columbia, Canada; 7 March 2013 in Seattle, Washington, USA; 22 May 2013 in Hong Kong, Special Administrative Region of the People’s Republic of China; and 19 July 2013 in Chicago, Illinois, USA.

There was no involvement of Baxter Healthcare or other external sponsors in the development of Global Forum resources. No direct financial support was provided to authors, other than for expenses relating to travel and accommodation for the aforementioned meetings. In the interest of transparency and full disclosure, con?ict of interest statements of all members appear in the following table.
